Cost of Passport form
Processing fee
The form must be submitted with 4 colour passport size photographs (full face), taken not more than three months from the date of application.
- Include the old passport
- Payment of fees should be made by money order or cashier's check, payable to "Ghana Consulate, New York". Applicants can also pay by cash if they submit their applications in person.
- A period of 3 to 4 weeks is required to process such an application.
Missing passports
The loss of the passport must be reported to the local police.
- Cost of Passport form- $20.00
- Processing Fee- $160.00
- The form must be submitted with 4 colour passport size photographs (full face).
- Original Police Report indicating loss of passport.
- Birth Certificate (both original and photocopy).
- Two other forms to be completed, one of which should be notarized.
- Photo ID (colour copy).
- Application would take 3 to 4 weeks to process.
Other Requirements
- Please return the completed application form along with a pre-paid USPS next day delivery envelope, for the return of issued passports.
